When Your Hips Don’t Lie: Understanding SIJ Dysfunction

What exactly is SIJ dysfunction?

SIJ dysfunction refers to pain stemming from abnormal movement (too much or too little) or misalignment of the sacroiliac joint – the connection between your sacrum and your pelvis. Despite its tiny range of motion, this joint plays a pivotal role in transmitting forces between the upper body and lower limbs, acting as a shock‑absorber. Research suggests that SIJ dysfunction may account for 15–30% of mechanical low back pain cases.

 

Who’s at risk – and why?

Repetitive micro‑trauma, acute trauma, pregnancy (thanks to the hormone relaxin), advancing age, and post‑lumbar surgery are all risk factors for SIJ dysfunction. Daily habits can contribute, like always carrying your bag on one shoulder, leaning on one hip while standing, or sitting for long stretches. These patterns create uneven stress across the pelvis and spine, making the SI joint more prone to irritation.

What does SIJ dysfunction feel like?

SIJ dysfunction often shows up as a stubborn ache in the lower back or buttock, usually on one side. Pain can radiate into the thigh or groin, and is often triggered by standing up from sitting, climbing stairs, bending, or twisting. Unlike sciatica, SIJ pain rarely travels below the knee.

What will your chiropractor do?

Chiropractors use a thorough assessment, including movement analysis and specific tests, to identify SIJ dysfunction. Treatment may include gentle manual therapy to restore motion, targeted exercises to strengthen supporting muscles, and strategies to improve posture and daily movement habits.

Prevention and long-term comfort

Keeping the SI joint happy means maintaining strength and mobility around the pelvis and core, varying postures through the day, and avoiding habits that overload one side of the body. Regular breaks from sitting, balanced lifting techniques, and switching shoulders for your bag are simple ways to reduce strain. Listening to early warning signs and seeking support can prevent a niggle from becoming a long-term issue.
